What Are Dermal Fillers?
Dermal fillers are gel-like substance inserted under the skin via injection to revive fullness and volume to the face. The fillers are made-up of different substances and categorized by the compound they are made up of. Each filler is specifically designed to target patient concerns such as:
- smoothing age lines
- restoring sunken cheeks
- plumping lips
- fading crease lines around the chin, eyes, and mouth,
- restore hollow under-eye areas
- lift cheekbones

How do fillers work?
Fillers can be made from natural or synthetic materials and are designed to target specific concerns and areas on the face. It is important to note that the fillers used at the Sadeghi Center for Plastic Surgery are FDA-approved.
Among the more common choices for dermal fillers is hyaluronic acid, which is a natural compound that retains water in our skin, eyes, and connective tissue to retain moisture and keep our skin hydrated. However, as we age and expose our skin to the sun, our naturally occurring hyaluronic acid decreases and loses its structure. When used as a filler, hyaluronic acid is injected under the skin as a smooth gel to fill depressions and lines. It is usually already mixed with lidocaine, which causes numbness, for a more comfortable treatment.
Another natural filler called Calcium hydroxylapatite, which is found in our bones, is a thick calcium-based gel that works to smooth deep wrinkles and also stimulates the production of collagen. It plumps the face, giving it back its vibrancy and adds volume to the area between the nose and mouth in which crease lines can naturally occur.
Polylactic acid is a filler (synthetic) which is safe and compatible within our bodies. It is usually injected for use on deep wrinkles and to plump skin. Polylactic acid actually stimulates the natural production of collagen and helps our skin rebuild collagen. Due to the way it works, results are gradual as the body begins to produce more natural collagen. Polylactic acid is also a biodegradable filler which can last for over two years. So you don’t need to revisit the doctor as often as you would with other treatments.
Another synthetic filler is Polymethyl methacrylate microspheres. These are long-lasting fillers that looks like tiny balls. The filler is injected under the skin and remains there. It contains collagen to provide a firm structure as one of the more durable biodegradable fillers on the market.
Types of Fillers
Juvederm™ Voluma, Juvederm™ XC, Juvederm™ Voluma XC
As the number-one dermal filler brand, Juvederm is the favorite filler of physicians and healthcare professionals worldwide. Juvederm XC is infused with lidocaine for a more comfortable treatment experience.
This patented filler is designed to restore volume to the cheek area, the primary area of the face that loses volume as we age. The natural curve of the cheek is a subtle indicator of youth and can play a key role in highlighting your beauty.
This filler works very well on nasolabial (mouth- and nose-area wrinkles). It works immediately, so that even as you leave Dr. Sadeghi’s office, you’ll begin to see the results of your treatment.
Restylane was the first hyaluronic acid-based dermal filler to be FDA-approved for use in the U.S. Often used to plump up lips, Restylane can also be used around the mouth to diminish smile lines; around the eyes to reduce the appearance of crow’s feet; and to restore volume to hollow areas in the cheeks or under the eyes.
Another of our favorite fillers to add volume and plump the skin, Radiesse is made of calcium-based microspheres instead of hyaluronic acid. In addition to being immediately effective, the treatment helps stimulate your skin’s natural collagen production — extending its effects. It’s especially effective in deep nasolabial folds, and in “parentheses” around the mouth.
How Long Do Filler Results Last?
Results usually depend on the patient, the type of treatment the patient has received, and the area that has been treated, but there are some general timeframes that can help a patient understand how long certain fillers will last compared to others. Natural fillers, such as hyaluronic acid fillers, can last anywhere from six to eighteen months. Although fillers in the lips and between the mouth and nose may fade faster, so you may have to return to maintain results.
Synthetic fillers can last longer because they are not absorbed by the body like natural fillers. Depending on the where your procedure has occurred, synthetic fillers can last several years.

How the Procedure Works
When you are ready for your procedure, the area to be treated will be cleaned and numbed with an anesthetic before you receive your dermal filler injection. You will be injected with a specific amount of filler beneath the skin, depending on dermal filler-type and product. Usually, the denser the filler, the deeper the gel is inserted into the skin. The procedure is quick and results are usually immediately noticeable.
While your surgeon may ask you to stay away from strenuous activity for a day or so, you can go back to your normal activities after the procedure. Swelling and bruising may occur at the site of injection, and numbness can also be a side effect after treatment, but is usually temporary.
Some patients, depending on filler-type, can experience bumpy skin at the site of injection, but with the use of hyaluronidase, the bumps can be smoothed out.
